This role sits at the core of the platform — the integration layer connecting hospitals, clinics, and EHR systems , where reliability directly impacts clinical workflows . This is not just backend engineering — this is building the nervous system of healthcare AI systems . What You'll Do Design and own complex integration systems (EHR / EMR / APIs) Map and understand undocumented or legacy systems Build reliable backend systems with: Async patterns Queues Observability Own production: Runbooks Incident handling System reliability Mentor engineers through code reviews and technical guidance Engage with open-source ecosystems (debugging, contributing, extending) Tech Stack Node.js, TypeScript, Python PostgreSQL, MongoDB REST APIs, GraphQL What We're Looking For 5+ years of backend engineering experience Strong expertise in: Node.js + TypeScript Python Deep understanding of: Async systems Queues Error handling Observability Experience building integration-heavy systems Comfortable working in ambiguity with high ownership Bonus: Healthcare / EMR / EHR integrations REST + GraphQL API design Open-source contributions Startup experience Compensation & Benefits $100,000 – $180,000 (USD) High-impact role in a fast-growing healthtech company Strong engineering culture with real ownership Location & Eligibility Remote (Brazil — São Paulo-based preferred) Must be based in Brazil Must have valid work authorization (no visa sponsorship) Ideal Candidate Profile Has built complex integrations in real-world systems (health, fintech, etc.) Thinks in systems, reliability, and edge cases Comfortable navigating uncertainty and undocumented environments Strong ownership mindset — builds, ships, and maintains Why This Role Work on mission-critical systems used in real clinical environments Build complex integrations across fragmented healthcare systems High ownership: shape architecture, not just execute tasks Direct impact on patient workflows and medical operations Join a high-agency, fast-moving engineering culture If you want to build systems where reliability isn't optional — and where your work directly impacts real clinical environments — this is where you make that impact.
